• If you are a clinic and use the application form: the clinic and contact details and the licenses you list.
• Standard technical logs kept by our hosting provider.

Please do not send medical records

We are not a medical provider. Please do not put diagnoses, X-rays or other health records in the form.

How we use it

• We use your details only to answer your request.
• We do not sell them.
• We do not give a patient’s details to any clinic.
• Requests are stored in our database and an email alert is sent to us.

Your choices

You can ask us to delete your request by writing to us through the contact form on this site and giving the email address you used.
